A Customizable Soccer Lesson Payment Form
Use WP Simple Pay to create a soccer lesson payment form and collect payments for your soccer lessons, group lessons, or private classes.
Collecting payments as a soccer instructor requires collecting customer identification information, type of lesson, and the month of year the payment is for.
Once you have selected the Soccer Lesson Payment Form template in WP Simple Pay, you can then customize your form field options and add as many extra fields as you’d like. The template includes the following fields by default:
- Student Name
- Student Email Address
- Student Phone Number
- Lesson Type (price selector)
- Month of Lessons (dropdown field)
- Credit Card
Additional options you may add are custom fields such as a “Special Focus” multi-line text field, or opt-in to automatic billing with recurring payments.
