A Customizable Martial Arts Lesson Payment Form
Do you run a martial arts dojo, or teach self defense classes? Use WP Simple Pay to easily accept online payments from your students.
Once you have selected the Martial Arts Lesson Payment Form payment form template in WP Simple Pay, you can then customize your form field options and add as many extra fields as you’d like. The template includes the following fields by default:
- Student Name
- Student Email Address
- Student Phone Number
- Lesson Type (price selector)
- Month of Lessons (dropdown field)
- Credit Card
Additional options you may add are custom fields such as a “Special Focus” multi-line text field, or opt-in to automatic billing with recurring payments.
